How to Submit a Histopathology Study

To submit tissues to CBI for slide preparation or study evaluation, or to submit slides for pathologic evaluation, please download our form (MS Word, 120K) , fill it out, print it and email or fax it to us.  CBI will be happy to provide a quotation or estimate for any study upon request.

• Following the in life phase of a study, the sponsor submits the tissues or slides to the Histology Department at CBI. 
• As soon as the tissues or slides are received at CBI, the sponsor is notified immediately of the submission’s safe arrival.
• A strict chain of custody is maintained.
• Each study is given a unique study number and each client is given an identification code to maintain confidentiality.
• The submission is inventoried and accessioned.  Any discrepancies are reported to the sponsor immediately. 
• Confidentiality is maintained.
• The slide study is prepared promptly and according to protocol or sponsor’s instructions.
• There is 100% slide quality control check and there is no charge for quality recuts.
• At the end of the study, the materials are promptly returned to the sponsor.
• Information is released only to sponsor or sponsor’s designee.
• Wet tissues may be bagged and prepared for permanent archiving upon request.
• All studies are performed within the CBI facility under the direction of CBI staff; our studies are never outsourced or subcontracted.
